Background: Managing HIV treatment is a complex multi-dimensional task because of a combination of factors such as stigma and discrimination of some populations who frequently get infected with HIV. In addition, patient-provider encounters have become increasingly multicultural, making effective communication and provision of ethically sound care a challenge. Purpose: This article explores ethical issues that health service providers in the United States and Botswana encountered in their interaction with patients in HIV care. Research design: A descriptive qualitative design was used (...) to collect data from health service providers and patients using focused group discussions. This article is based on responses from health service providers only. Participants and context: This article is based on 11 focused group discussions with a total sample of 71 service providers in seven US sites and one Botswana site. Ethical considerations: Ethical review boards at all the study sites reviewed the study protocol and approved it. Ethical review boards of the study’s coordinating centers, Rutgers University and the University of California at San Francisco, also approved it. The study participants provided a written informed consent to participate. Findings: HIV service providers encountered ethical challenges in all the four Beauchamp and Childress’ biomedical ethics of respect for patients’ autonomy, beneficence, justice, and nonmaleficence. Discussion: The finding that HIV service providers encounter ethical challenges in their interaction with patients is supported by prior studies. The ethical challenges are particularly prominent in multicultural care and resource-constrained care environments. Conclusion: Provision of HIV care is fraught with ethical challenges that tend to pose different issues depending on a given care environment. It is important that strong partnerships are developed among key stakeholders in HIV care. In addition, health service providers need to be provided with resources so they can provide quality and ethically sound care. (shrink)

It is argued that the so-called fitting attitude- or buck-passing pattern of analysis may be applied to personal values too if the analysans is fine-tuned in the following way: An object has personal value for a person a, if and only if there is reason to favour it for a’s sake. One benefit with it is its wide range: different kinds of values are analysable by the same general formula. Moreover, by situating the distinguishing quality in the attitude rather than (...) the reason part, the analysis admits that personal value is recognizable as a value not only by the person for whom it has personal value, but for everyone else too. We thereby avoid facing two completely different notions of value, viz., one pertaining to impersonal value, and another to personal value. The analysis also elucidates why we are justified in our concern for objects that are valuable for us; if value just is, as it is suggested, the existence of reasons for such a concern, the justification is immediately forthcoming. Chemical structures are among the trademarks of our profession, as surely chemical as flasks, beakers and distillation columns. When someone sees one of us busily scribbling formulas or structures, he or she has no trouble identifying a chemist. Yet these familiar objects, which accompany our work from start to end, from the initial doodlings (Fig. I) to the final polished artwork in a publication (Fig. II), are deceptively simple. When viewed the history of Islamic thought, it is seen that the scholars have made an effort to understand the nature of the speech of God and make sense of it. Essentially, understanding and grasping of the words of God are an effort to look from the physical realm to the metaphysical one. In spite of this fact, the scholars as the indomitable seekers of truth are in search of finding some clues to say about it. While some of them (...) consider the “divine speech” as an attribute of God, many others view it as a divine act. It is also admitted by all of them that whether being attributive or act of God, the reflection of the divine speech is the Qur’ân. In this sense, three views have been put forward on the question of the belonging of the Qur’ânic words. According to the dominant and preferred point of view among these views, Qur’ân belongs to God in terms of wording and meaning. Accordingly, the revelation of the Qur’ân, which was revealed to the Prophet Mohammed consists of both wording and meaning. Therefore, there is no intervention or contribution of Gabriel or Muhammad in constituting of Qur’ân. According to the second approach, the meaning of Qur’ânic text belongs to God, however, its words pertain to Mohammed. This view signals that Gabriel revealed nothing more than the meanings of Qur’ân. Mohammed understands the meanings conveyed to him and then said them in Arabic expression patterns and phrases. According to the third approach, the meanings of Qur’ânic text belongs to God, however, its words pertain to Gabriel. In other words, only meanings of text were given to Gabriel, and its words are formed by him. In the present study, the mentioned approaches which are put forward by scholars about the belonging of Qur’ânic words will be examined in terms of the integrity of the Qur’ân and Tafsir rumors (riwâyât) of the classical period. In a nutshell, it can be said that there is no disagreement among scholars in the field of the belonging of the meanings of Qur’ânic text to God. The main disagreement is whether the Qur’ânic words belong to God, Gabriel or Muhammad. Considering the integrity of Qur’ân and Tafsir rumors regarding to the circumstances of sending down of the Qur’ânic revelation, it has been determined that the prevailing view is more consistent than the other two views. The experience of newborn screening for Krabbe disease in New York State demonstrates the ethical problems that arise when screening programs are expanded in the absence of true understanding of the diseases involved. In its 5 years of testing and millions of dollars in costs, there have been very few benefits, and the testing has uncovered potential cases of late-onset disease that raise difficult ethical questions in their own right. For these reasons, we argue that Krabbe screening should only be (...) continued as a research project that includes the informed consent of parents to the testing. (shrink)

This distinction has sometimes been read as purely epistemic, resting not on things, but on our knowledge of them: there is one world, not two, though it may be apprehended in two ways. But this view is patently at odds with the text. Knowledge and opinion are δυνάμεις, "faculties," to be distinguished and defined by their objects, no less than by the state of mind they produce, and Plato clearly states that the fallibility and unclearness of opinion is rooted in (...) the relative unreality of opinables. In his article ‘A Cavalry Unit in the Army of Antigonus Monophthalmus: Asthippoi’, N. G. L. Hammond argues that the reading of the manuscript R at Diodorus 19. 29. 2 should be retained and that we should read ⋯π⋯ π⋯σι δ⋯ το⋯ς τε ⋯σθ⋯ππους ⋯νομαζομ⋯νους κα⋯ τοὺς ⋯κ τ⋯ν ἄνω κατοικο⋯ντων ⋯κτακοσιο⋯ς. The readings of F and its copy X, ⋯νθ⋯ππους, and the commonly accepted conjecture of Wesseling ⋯μɸ⋯ππους, should both be abandoned. Hammond's arguments for retaining this reading are that (...) between the variant readings of R and F, R ‘is the more often correct’ ; ⋯σθ⋯ππους, the reading of R, is preferable to F's ⋯νθ⋯ππους on the principle of ‘lectio difficilior’; the ‘difficulty’ of his lectio difficilior is made less by the arguments presented by A. B. Bosworth, 245 ff.), for the restoration at several places in the text of Arrian's Anabasis the word ⋯σθ⋯ταιροι, in place of the editorial emendation of πεζ⋯ταιροι. ‘For just as asthetairoi meant an élite group of Macedonian infantrymen,’ says Hammond, ‘so asthippoi should mean an élite group of Macedonian cavalrymen.’ He then proceeds to examine the passage and its context in an attempt to find confirmation of this proposition and comes to the conclusion that Diodorus' text says that there were 800 cavalrymen, broken into two groups, the asthippoi and ‘the men from the up-country settlers’; these, though having different names, had a close relationship to each other, as is shown by their being brigaded together. Following his derivation of asthetairoi as meaning ‘townsmen-companions’, i.e. companions recruited from the towns of Upper Macedonia, he argues that asthippoi were cavalry recruited from the towns of Upper Macedonia and that τοὺς ⋯κ τ⋯ν ἄνω κατοικο⋯ντων means ‘the sons of settlers in up-country Macedonia’. These 800, says Hammond, were both among Antigonus' best troops and ‘were Macedonians from Europe’. We thus appear, on Hammond's interpretation, to have here a group of 800 Macedonian élite cavalrymen, all of whom were recruited in Upper Macedonia's townships, but of whom some were chosen to be the cavalry-equivalent of the élite ‘townsmen-companions’ by their title of ‘townsmen-cavalry’, whilst the others, similarly recruited – but perhaps of a younger generation? – had no particular distinguishing title beyond ‘sons of settlers in up-country Macedonia’.Now, while it would be exciting to be able to add another one, possibly two Macedonian cavalry units to the Macedonian army of Philip, Alexander and the Successors, there are several aspects of Hammond's arguments that, it seems to me, are less than convincing. Thus, whilst it seems beyond reasonable doubt that ⋯σθ⋯ταιροι should be retained in the text of Arrian, appearing, as it does, six times, this is not an argument that ⋯σθ⋯ππους should be read in Diodorus' text. Both it and F's reading of ⋯νθ⋯ππους are hapax legomena and it is possible that both readings are wrong. But even if we retain ⋯σθ⋯ππους as the correct reading, Hammond's explanation of its meaning is open to objection on several counts. (shrink)

The mutakallimūn, who began to take care of nature as a result of their metaphysical concerns from the early period and with the influence of the dualist and materialist groups, suggested various theories that attempt to explain the structure and functioning of the universe. Over time, many subjects of physics became an indispensable part of Kalām and were used in the proof of the fundamental principles. Thus, in addition to the definition of body (jism), Kalām books began to contain detailed (...) topics such as time, space, movement and distance. After summarizing the definitions of body, which were made by the mutakallimūn, this article will examine the contents of Ibn Kamāl's epistle on ṭafra (leaping), which has many copies with various titles in the Turkish Libraries. In the beginning of the epistle, Ibn Kamāl mentions the descriptions of body suggested by the prominent mutakallimūn, and then, he compares the views of al-Naẓẓām, who believes in the finite division, with atomist mutakallimūn who accept the infinite division. He criticizes the idea of ṭafra with regard to the notion of body in Kalām. After explaining the arguments of both groups with their justifications, he points out the weaknesses of the arguments of each group, as well. In this connection, he revisits the theory of ṭafra, which is, he argues, a mandatory consequence of al-Naẓẓām’s definition of object, as he also reviews the opinions of Quṭb al-Din al-Rāzī in this direction. We generalize two well-known model-theoretic characterization theorems from propositional modal logic to first-order modal logic. We first study FML-definable frames and give a version of the Goldblatt–Thomason theorem for this logic. The advantage of this result, compared with the original Goldblatt–Thomason theorem, is that it does not need the condition of ultrafilter reflection and uses only closure under bounded morphic images, generated subframes and disjoint unions. We then investigate Lindström type theorems for first-order modal logic. We show that FML has (...) the maximal expressive power among the logics extending FML which satisfy compactness, bisimulation invariance and the Tarski union property. (shrink)

Abū Isḥāq Ibrāhīm al-Zāhid al-Ṣaffār is one of the important representatives of the Kalām thought of the Ḥanafī-Māturīdī School. He was born in A.H. 450/ C.E. 1058 in Bukhara that is one of the city of Transoxiana/Mawara al-nahr where Maturidi School has emerged around this socio-cultural environment. He completed his education and served as a leader (rais/ ṣadr) of the Hanafite scholars there until he was exiled to the city of Marv. Upon his return from exile, he passed away in (...) Bukhara in A.H. 534/ C.E. 1139. The present study aims to determine his method in Kalām. The introduction outlines the subject, significance, purpose, method, and sources of the research. In this study, it was employed a method that focuses on a person in which he/she is examined with his/her views and thoughts by taking into account his/her socio-cultural factors. By doing this, the first chapter of this study deals with the political, socio-economic, and scientific situation in Transoxiana, and the activities of diverse religions, sects, and philosophical schools in this region in A.H. V-VI/C.E. XI-XII. In the second chapter, it studies al-Ṣaffār’s life, scientific personality, works as well as the sources that he made references to in his works in detail. In the third chapter, the kalām method of al-Ṣaffār has been tried to be determined. For this purpose, it analyzes his extant works, Talkhīṣ al-adilla li-qawāʿid al-tawḥīd and Risālah fī al-kalām and elaborates first on his defense of the Kalām that gives great importance to reason (ʿaql) and to intellectual debates, if necessary, in theological issues. Then it explores his theory of knowledge with his ideas on the definition, possibility, sources, and types of knowledge, and the epistemological understandings he defensed or criticised. Furthermore, to determine his Kalām method, it evaluates his approach towards the value of ‘aql and naql (Qurʾān-Ḥadīth) as sources of knowledge. Since this issue is a distinctive tool for unveiling the kalām methods of different mutakallims and Muslim sects. Finally, it finds out the types of istidlāl, meaning inference of one thing from the other that he used when he argues and defends his views, and illustrates with examples. (shrink)

On May 24, 1995, the American Medical Association Council on Ethical and Judicial Affairs issued a rather controversial opinion that it is ethically permissible to use anencephalic infants as living organ donors. Approximately 1,000 to 2,000 infants are born each year in the United States with anencephaly, a congenital birth defect whereby the infant has no forebrain and cerebrum. Without higher brain functions, the infants can never experience consciousness, thoughts, emotions, or pain. Fewer than half survive more than a day, (...) and more than 90 percent die within one week. However, by the time brain death occurs in anencephalic infants, their organs are no longer suitable for donor purposes because they have deteriorated beyond use.In 1988, the AMA Council declared that it is ethical to use the infants as organ donors only after they have died. Now the Council has revised that opinion, and it proposes to allow removal of organs before death, as long as the parents consent and two doctors concur in the diagnosis of anencephaly. (shrink)

In this paper, I use insights from post-structuralist anarchism to consider the relationship between a sense of the future, or “futurity”, and the notion of utopia for anarchist movements. At issue is whether anarchism requires a vision or sense of the future at all and, if so, whether that futurity should be utopian. Drawing from the post-structuralist anarchism of Todd May, Saul Newman, and Lewis Call, I consider the problems with utopia, as well as the potential irrelevance or impossibility of (...) even thinking the future. I then argue for the necessity of both and contend that post-structuralist anarchism does not preclude either futurity or provisional forms of utopia. I conclude by sketching the outlines of a utopia that would be acceptably post-structuralist and acceptably anarchist. (shrink)

Theoretical problems concerning concepts of systems and measurement of behavior were encountered during experimental studies of the effects of electrical stimulation of the brain on the social behavior of a monkey colony. General problems involved in the description and measurement of behavior of natural systems, and especially of organisms are discussed. In animals with differentiated brain the general process of stimulation may be divided into four subprocesses: input, throughput, transput and output. Categories of behavior, temporal and spatial units, and logical (...) rules of homogeneity for sorting data and comparing measurements and categories, are discussed. Simple behavioral units are divided into: 1. Individual a. static or postural b. dynamic or gestural 2. Social a. static b. dynamic. Complex behavior is divided into: 1. Simultaneous 2. Sequential 3. Syntactic 4. Roles. The process of interpretation of data by the observer is assumed to be dependent upon actual behavior of the system, techniques of observation and experimentation, theoretical and technical processes of analysis, and synthesis and integration of data. Expression of behavior in mathematical terms insures greater accuracy than descriptive methods and enables data to be processed with computers. (shrink)

Abu Ishaq al-Saffar was one of scholars of the Western Qarakhanids' period who followed the Kalam thought of al-Maturidi (d. 333/944). His theological works Talkhis al-adilla and Risala fi al-kalam, his method in kalam, and frequent reference to his works by Ottoman and Arab scholars indicate that al-Saffar is a respected and authorative Maturidi theologian. The article focuses on his defense of the kalam. By adding a long introduction to Talkhis about the naming, importance, and religious legitimacy of the science (...) of kalam, Saffar asserted that the kalam should be learned. When systematically vindication of the science of kalam is examined, it is understood that al-Saffar is the first theologian who reserved a private and voluminous part for defending the kalam among Hanafi-Maturidis. Even though he does not state its systematic, it can be understood that vindication of kalam in al-Saffar divides into three parts as of explanation, demonstration and refutation. Al-Saffar's defending the science of kalam shows that there were opposite thoughts against kalam in the 4th/10th and 5th/11th centuries throughout in Transoxania. In this period Ashab al-Hadith and Ikhwan al-Safa were the opposite fronts of kalam. In addition, he was trying to go beyond the oppositions of Hanafi jurists. His vindication is consistent and has scholarly depth because it is able to be against a strong opposition. (shrink)

Daniel Dennett and others have suggested that qualia and introspectible phenomena do not exist. Dennett's account of consciousness, along with several related approaches, has been called illusionism by Keith Frankish. Frankish's analysis is helpful and provocative. As currently presented, however, his 'strong' version of illusionism suffers from several basic confusions, particularly regarding its relationship to eliminative materialism. This paper contrasts strong illusionism with an alternative that is easier to understand and more sharply focused -- fallibilist experiential realism, or, less technically, (...) modest realism. Fallibilist realism affirms the existence of experiential states that are commonly considered qualitative and phenomenal, but shares Frankish's agenda of thoroughly re-evaluating our judgments about such experiences. To illustrate the advantages of modest realism, I compare the ways illusionism and modest realism address two challenging puzzles: explaining how qualitative differences among sensory experiences could be constituted by differences among neural states, and explaining how neural states could constitute any conscious experiences whatsoever. A fitting-attitude analysis which understands value in terms of reasons and pro- and con-attitudes allows limited wriggle room if it is to respect a radical division between good and good-for. Essentially, its proponents can either introduce two different normative notions, one relating to good and the other to good-for, or distinguish two kinds of attitude, one corresponding to the analysis of good and the other to good-for. It is argued that whereas the first option faces a counterintuitive scope issue, an (...) attitudinal approach couched in terms of ‘for someone's sake’ attitudes has the unwelcome consequence that whatever is good for someone is also necessarily good. It is argued that this consequence can be avoided if we modify the standard way of formulating the fitting-attitude analysis of final impersonal value. (shrink)

A prolific discourse on the sexualisation of girls has developed in the Anglophone west. Since 2006, at least six governmental policy papers, four think tank reports, ten parenting manuals as well as over a thousand newspaper articles have been published on the topic. Deconstructing popular feminist narratives, one finds that beneath calls for protection there often resides a deeply ambivalent construction of the middle-class white girl. I argue that these narratives are beset by a melancholic subtext, one that is fuelled (...) by care and a desire for the future as well as an undercurrent of suspicion, anger and resentment. in UndeterminedClassical fitting-attitude analyses understand value in terms of it being fitting, or more generally, there being a reason to favour the bearer of value. However, recently such analyses have been interpreted as referring to two reason notions rather than only one. The general idea is that the properties of the object provide reason not only for a certain kind of favouring vis- à-vis the object, but the very same properties should also figure in the intentional content of the favouring; (...) the agent should favour the object on account of those properties that provide reason for favouring the object in the first place—where “favouring on account of” refers to the agent’s so-called motivating reason. This paper discusses this novel approach to fitting-attitude analysis: should those considerations that constitute the reason for favouring also be included in the intentional content of the favouring. In other words, should fitting-attitude analysts require a reference to the agent’s motivating reason in the analysis? While this enlargement of the original proposal might seem intuitive given that favourings are discerning attitudes, it is nonetheless argued that proponents of the fitting-attitude analysis are in fact not served by such an expansion of the classical analysis.
